Rural Medical School Keeps Doctors "We need more doctors. A quarter of all of our physicians in Kansas are 60 years or older. So we need to be replacing physicians too," says Dr. William Cathcart-Rake, who directs a new program at Kansas University which is designed to provide those physicians. The idea is to get medical school students to practice in rural Kansas by educating them in the state's smaller communities. According to Cathcart-Rake, students from rural areas typically go off to medical schools in Wichita and Kansas City. "They say they have every intention of coming back to rural Kansas but they meet a soul mate, they get married. Their soul mate happens to be from a big city and we never see them agai. They get captured in the big city," he says. "Hopefully, if we train them in smaller communities, they can meet their prospective spouses here, they can network here and they have those connections which can be lifelong." The program is based in the small town of Salina - population 50,000 - which is about a three-hour drive from Kansas City, past corn and soybean fields and the occasional cattle ranch. The medical school in downtown Salina is housed in a three-story brick building which was refurbished specifically for the program. The first thing you notice is that the professor is not in the room. Instead, he appears on a flat-screen monitor at the front of the classroom. "It's set up with interactive TV, so we have two monitors. On one, we see the lecturer's slides, and on the other monitor, we see video of the lecturer," says Claire Hinrichsen, a student in the Salina program. "Most of our lecturers are out of Kansas City. Today, our lecturer was in Wichita." The professor in Wichita also has a video feed of the Salina classroom. When students have a question, they raise their hand and speak into the microphone built into their desk. Hinrichsen grew up in Lecompton, Kansas, a town of about 600 people. One of the reasons she chose the Salina program is because of the size of the class. There are only eight students, making it the smallest medical school in the country. "I really like it. I know everybody in my class," she says. "We are close and it's a nice feeling. It goes back to when I went to KU (Kansas University), so I went to a big school, and I'm getting back to the small feel." Other classes are taught by professors in Salina, and students will get hands-on training throughout the four-year program. Then each one will go on to a residency in a small community in the surrounding area. Dr. Kerry Murphy is a family physician at the Clay Center Clinic, about 100 kilometers northeast of Salina. Chances are at least one of the eight students in the program will do a residency here. "This is a clinic that has currently eight doctors and four mid-level practitioners and we cover this town, but we also have satellite clinics in two nearby towns," Murphy says. "We operate as what I call a cradle-to- grave operation, we deliver babies and go up to doing nursing home care." It's unlikely that there will ever be enough primary care physicians for every small town to have its own doctor. Cathcart-Rake expects several doctors will work out of a central clinic, like Clay Center, and see patients at satellite clinics spread out around the county, like spokes of a wheel. "Clay Center is a great model for what I foresee as what a primary care center in Kansas might be," he says. At the end of their residency, the students will have spent six years in rural communities in Kansas. Cathcart-Rake hopes that will be enough time for them to put down roots. "And it's gonna hopefully harder for them to break away from those roots and go to bigger cities." One thing that could lure them away is money. Rural doctors typically serve older, poorer patients and going into a specialty practice in a big city tends to be far more lucrative. And with the high cost of medical school, it's difficult for graduates to turn down those opportunities. So the Salina program is offering extra financial incentives to help them resist. "It's called the KMS, the medical student repayment program," says Claire Hinrichsen, one of the five medical students who signed up for it. "For every year you give back to a rural area they pay for all our tuition." That means, if they practice in an underserved county for four years, they will have essentially gotten their medical degree for free. But, Cathcart-Rake says there's a catch. "If they don't practice primary care in an under-served area, they have to pay that back, with interest. So they have got those hooks in them. We hope this innovative program will keep students close to their roots. But only time will tell."

Americans were surprised to find their country rated 13th in quality of life. Finding that hard-to- define measure of satisfaction has been a big issue in this country. We search for such things as good schools for our kids, personal space and access to nature. We move incessantly, sometimes for reasons no more profound than to be close to a favorite trout stream or sophisticated nightlife; or to have a waterfront view, a yard big enough for the dog to run, or a welcoming church or synagogue or mosque. Of course, quality-of-life considerations must sometimes give way to job demands or the needs of a spouse or parent or child. But whenever we can, we try to adjust our recipes for a better life, perhaps building a den for reading or moving to a leisure village with a golf course. The American West has been settled; the wild places tamed. The search for quality of life is Americans' new,personal, ever-changing frontier. And, just as in beautiful Ireland or Italy or Spain, tough times have made our search for the good life more challenging of late. It would be interesting, in fact, to see where the United States would rank if a worldwide quality-of-life survey were conducted in 2012. A possible indicator, though - and not a very promising one - is the Mercer Human Resources Group's worldwide quality-of-living ranking of cities, completed last year. Only six U.S. cities made the list of the world's 50 most agreeable places to live. Meanwhile, small fires have been lit in the square in the hope that the rising smoke will carry the tear gas with it. Cecilia Perry has been coming to this shop, Chocolate Chocolate, almost every day for years. "I come here often because they have the best chocolate in the entire region ," said Perry. Esther Alpert comes in a couple of times a week. "It is just so sweet and pleasant to be here. We love these guys, and I think it is the best chocolate in the city," Alpert added. Offering the best chocolate from around the world is what Frances and Ginger Park had in mind when they opened the first independent chocolate shop in the nation's capital 28 years ago. "We grew up on some very fine chocolate because our dad worked for the World Bank and would bring back European chocolate in suitcases when we were very young," said Frances Park. When their father died in his mid-50s, the sisters grew closer and decided to go into business together. "But we did not know what we were doing and we definitely ate more chocolate than we sold that first year," Frances Park added. "But we learned one thing we did was we sort of over time created family with our customers." That family has grown and includes long-time customers like Beth Smith. "I have been coming here for a long time, probably 20 years," noted Smith. "She remembers the birth of my daughter. She remembers all the, you know, I feel like family here." Sales have increased over the decades and, today, this little shop has four staff members... including Ginger's husband and a manager. "It is a huge success," said Ginger Park. "I mean certainly not on the level of Donald Trump, but for us it is a huge success. We wanted to build a life around our business and that is what we did." The Park sisters also wanted to feed their literary side. They've co-authored eight books for children and adults. Their first, award-winning book, My Freedom Trip, is based on their mother's escape from North Korea when she was a teenager. "We wanted to write the book as a tribute to our mother and her mother, and also perhaps to give children an idea of the price that other people had pay for freedom," said Frances Park. The Park sisters also published a memoir that chronicles their quarter century behind the chocolate counter. FRANCES PARK: "It is actually about the ups and downs of two sisters who have always been very close, who went from being very young girls to being not very young girls, but still smiling." GINGER PARK: "Everyone who comes into our shop tells us you have the perfect life, and we say yes, you know what, we do, we really do. They go hand in hand, books and chocolate." The United Nations says the number of Malian refugees fleeing the fighting between Tuareg rebels and the military has doubled over the past 10 days. It says more than 44,000 have crossed into neighbouring countries. Martin Plaut reports. People have been crossing in their thousands into Mauritania, Niger and Burkina Faso, where the UNHCR has been registering them. The conflict has been fierce with government forces using helicopter gunships in an attempt to retake towns seized by the rebels. Amnesty International described the fighting, which erupted in January, as the worst human rights crisis in northern Mali for 20 years. Italian police have arrested eight people in connection with a seizure of fake US treasury bonds. Police said the alleged crime posed a serious danger to the stability of the international credit system. Alan Johnston reports. This is a case of attempted international fraud on the most extraordinary scale. The police say that the $6tn worth of counterfeit US bonds were seized in Switzerland. They were found in fake American Federal Reserve security boxes in a raid in January. And now several arrests have been made in different parts of Italy. Investigators say the gang may have been planning to sell the bogus bonds to investors in emerging economies or to more mainstream brokers. The US Secretary of State Hillary Clinton and the EU foreign policy chief Catherine Ashton say they are cautiously optimistic that talks with Iran about its nuclear programme might resume. The assessment comes after Iran's nuclear negotiator Saeed Jalili sent a letter to Baroness Ashton saying Tehran was ready to restart discussions. Police in the United States have arrested a man who they say was planning a suicide bomb attack near the US Congress in Washington. American security officials say the man, who's of Moroccan descent, had entered the United States with a family member and had overstayed his visa. From Washington, here's Adam Brookes. The man was arrested close to the Capitol building, allegedly wearing a vest he thought contained explosives. In reality, the man had been monitored by the FBI for a year, and the explosives, supplied by undercover FBI agents, were fake. Officials said the man believed the undercover FBI officers he was dealing with were al-Qaeda operatives. They said there was never any danger to the public. The head of News Corporation, Rupert Murdoch, has told staff at the biggest-selling British tabloid, The Sun, that he's lifting the suspension of 10 journalists arrested over alleged bribery. Announcing that the paper would bring out an edition on Sundays, he said the staff had his unwavering support, but added that illegal activities would not be tolerated. Journalists were angry that information passed to the police by News Corporation had led to the arrest of colleagues suspected of paying police to divulge confidential information. World News from the BBC The Chinese Deputy Foreign Minister Zhai Jun has arrived in the Syrian capital Damascus in an effort to mediate a day after China voted against a second UN resolution condemning violence by the Syrian government. Anti-government activists say 40 people were killed on Friday, some by security forces who opened fire on protesters after Friday prayers. The World Health Organisation says new research into the H5N1, or bird flu virus, should not be published in full until further analysis of the risks is carried out . The researchers have modified the H5N1 virus so that it might be passed from human to human. Imogen Foulkes reports from Geneva. Many scientists believe research into bird flu is essential to develop vaccines and treatments, but public health officials fear a mutant virus created in the laboratory could become a weapon in the hands of terrorists. The meeting in Geneva agreed only that publishing research in full was preferable to developing a way to partially censor it, but no date has been set for publication. In the meantime, a moratorium on research into bird flu has been extended indefinitely. Police in Colombia have arrested five suspected left-wing rebels they believe were involved in the bombing of a police station two weeks ago. Eleven people died when a motorcycle packed with explosives went off outside the police station in Tumaco on the Pacific coast. Four of the suspects are accused of handling the explosives, a fifth with providing a safe house. Police accused all five of belonging to the country's largest rebel group, the Farc.
